Muscle Masters Therapy ("we", "us", or "our") operates from 56 Crystalhill Drive, Brampton, Ontario. We are committed to protecting the privacy of every client in accordance with Ontario's Personal Health Information Protection Act, 2004 (PHIPA), Canada's Personal Information Protection and Electronic Documents Act (PIPEDA), and the standards of practice set by the College of Massage Therapists of Ontario (CMTO).
1. Information We Collect
To provide safe, effective massage therapy we may collect:
- Contact details (name, phone number, email, address).
- Date of birth and emergency contact.
- Health history, current medications, injuries, and treatment goals.
- Insurance information where you request direct billing.
- Assessment findings, treatment notes, and consent records for each visit.
- Payment information processed by our secure payment providers.
- Website analytics (see Section 7).
2. How We Use Your Information
Your information is used only to:
- Book, confirm, and manage your appointments.
- Assess and deliver massage therapy treatment.
- Maintain the clinical records required by CMTO.
- Process payment and, where applicable, submit insurance claims on your behalf.
- Communicate appointment reminders and clinic updates you have opted in to.
- Meet our legal, regulatory, and professional obligations.
We do not sell, rent, or trade personal information.
3. Consent
By booking an appointment you consent to the collection and use of your information as described here. You may withdraw consent at any time, subject to legal and professional record-keeping requirements. Additional written consent is obtained before each treatment session.
4. Sharing Your Information
We only share your information with:
- Your insurance provider when you request direct billing or a receipt.
- Other health-care providers when you provide written consent (e.g., a referring physician).
- Our booking platform (Jane App) and payment processors, which store data on secure Canadian servers.
- Regulatory bodies (such as the CMTO) if required by law or in the course of an investigation.
5. Storage and Security
Clinical records are stored in Jane App, a health-care practice management system that uses encrypted Canadian-hosted infrastructure. Access is restricted to the treating Registered Massage Therapist. Paper documents, if any, are stored in a locked location. We retain records for the minimum period required by CMTO (currently 10 years after the last date of service, or 10 years after a client reaches the age of majority).
6. Your Rights
You have the right to:
- Access the personal health information we hold about you.
- Request corrections to inaccurate information.
- Withdraw consent for future collection, use, or disclosure.
- Make a complaint to our clinic or to the Information and Privacy Commissioner of Ontario.
7. Website & Cookies
Our website uses standard cookies and basic analytics to understand how visitors use the site and to keep it running smoothly. No personal health information is collected through the website itself. Booking requests are handled through Jane App, which is subject to its own privacy policy.
